Support Coordination:
Support coordination is provided to help you build the skills you need to understand and use your NDIS plan. Our support coordinator’s will work closely with you to connect you with a range of supports that increase your capacity to maintain relationships, manage services, build independence, promote community inclusion and improve your quality of life.
Specialist Support Coordination:
Specialist Support Coordination is provided to you when you are facing complex and challenging situations which require a higher level of specialist support.
As your Support Coordinator we will:
- Help you to build your capacity to understand and implement the funded supports in your NDIS plan.
- Help you to get the most out of your NDIS plan and ensure your funded supports are aligned with your needs, preferences and budget.
- Support you to build your capacity in exercising choice and control.
- Support you to connect to mainstream services.
- Support you to negotiate with providers to personalise your supports and services.
- Collaborate and develop service agreements between you and your providers.
- Build your independence and empower you to coordinate your own supports.
- Promote participation in your community.
- Support you with planning and preparing your NDIS plan review.
